Acknowledge that you are a sinner and ask God for forgiveness for your sins. Repentance is the key to a changed life.
Believe
Believe in your heart that Jesus died and rose again for you.
Confess
Confess with your mouth that Jesus is now your Lord.
Dedicate
Dedicate your life completely to Him.

"Father, I confess my sins and ask for Your forgiveness. I believe and confess that Jesus died and was raised from the dead. I accept Jesus as my Lord and personal Saviour. I dedicate my life completely to You. In Jesus' name, Amen."
